Founder of Prime Vitality Medical & Wellness PLLC, Vahid Moradi is a board-certified Family Nurse Practitioner with more than 35 years of experience in emergency and intensive care medicine — now bringing that depth of clinical judgment to personalized, whole-person telehealth.
Vahid spent his career at the bedside in the emergency department and ICU, caring for patients in their most critical moments. That experience shapes how he practices telemedicine today: calm, thorough, and unwilling to cut corners on a diagnosis. Age eligibility depends on the service requested, the patient’s needs, and the provider’s clinical judgment. Please contact the office before booking care for a minor so the practice can confirm whether the visit is appropriate.
What does “Prime Vitality” represent?
The name reflects the practice’s goal of helping patients protect their health, restore balance, and pursue lasting vitality through evidence-informed, personalized medical and wellness care.
What conditions may be evaluated?
Prime Vitality evaluates many common concerns that can be assessed safely by telehealth or during an in-person visit. Examples may include uncomplicated respiratory symptoms, allergies, selected urinary symptoms, minor skin concerns, cold sores, pink eye symptoms, medication-refill requests, and other non-emergency concerns.
What additional services are available?
Depending on medical history, examination needs, and clinical appropriateness, services may also include primary and preventive care, women’s health consultations, skin-condition care, hormone-replacement evaluation, personalized weight-management support, and travel-medication planning.
Can you prescribe medication?
When medically appropriate, prescriptions may be sent electronically to your preferred pharmacy. A prescription is never guaranteed and is based on the provider’s evaluation, your history, medication interactions, allergies, examination findings when available, and current standards of care.
Do you prescribe controlled substances?
Prime Vitality maintains a strict restricted controlled-substance prescribing policy and does not prescribe, refill, renew, replace, or manage any DEA Schedule I or Schedule II controlled substance, or any opioid medication classified under Schedule III — whether by telehealth or in-person. Do you offer hormone or weight-management treatment?
Prime Vitality offers evaluation and ongoing support for hormone health and weight management. Treatment may require laboratory testing, an in-person assessment, medical-record review, regular monitoring, and follow-up visits. Medication eligibility is determined individually; no specific medication or outcome is guaranteed.
Can you help with travel medicine?
Pre-travel consultations may address altitude-sickness prevention, malaria prevention, traveler’s diarrhea planning, and motion-sickness medication. Prime Vitality does not administer travel vaccines during a telehealth visit. Patients may be referred to a pharmacy, travel clinic, urgent care, emergency department, laboratory, imaging center, or specialist when needed.
What if telehealth is not appropriate?
If your symptoms require a hands-on examination, vital signs, testing, imaging, a procedure, or emergency evaluation, the provider will recommend the safest next step. Some concerns may be converted to an in-person visit or referred to another facility.
Can laboratory tests be ordered?
Laboratory testing may be ordered when clinically indicated and when ordering arrangements are available. Patients receive instructions about where to complete testing and how results and follow-up will be handled.
When should I seek emergency care?

What if I do not have insurance?

Can I receive a receipt?

Where must I be located during the visit?
You must tell the provider your physical location at the start of the visit. Care can be provided only where the provider is legally authorized to practice and where the requested service is appropriate.
Can you provide a work or school note?
A brief work or school excuse note may be provided when medically appropriate and supported by the evaluation. Notes are not guaranteed, are generally not backdated, and reflect only the condition and time period clinically supported by the visit.
Do you complete FMLA, disability, accommodation, or return-to-work forms?
These forms often require an established treatment relationship, detailed functional assessment, records review, ongoing management, or an in-person examination. Prime Vitality Medical & Wellness PLLC maintains a strict restricted controlled-substance prescribing policy.
The practice does not prescribe, refill, renew, replace, or manage any DEA Schedule I or Schedule II controlled substance, or any opioid medication classified under Schedule III, whether the patient is evaluated through telehealth or during an in-person visit.

- DEA Schedule I controlled substances
- DEA Schedule II controlled substances
- Schedule II opioids and narcotic pain medications
