Activation and renewal
Membership begins after the first qualifying provider consultation, which is included in the first month’s membership fee. The initial commitment is three months, followed by month-to-month automatic renewal. If the provider finds membership clinically inappropriate before activation, no enrollment membership or consultation charge is due.
Cancellation and refunds
Written cancellation notice is generally required at least 30 days in advance. Early voluntary cancellation during the initial commitment generally does not eliminate owed fees, subject to the agreement and applicable law. Prorated refunds and commitment waivers apply in specified circumstances, including clinical inappropriateness or material inability to deliver services.
Payments and changes
Membership is not health insurance. Recurring payment authorization is required. The rate is fixed during the initial commitment. Afterward, routine rate increases require at least 30 days’ written notice, are limited to once per rolling 12 months, and cannot exceed 10% of the prior rate, subject to applicable law.
Care and exclusions
Membership does not guarantee medication, prescriptions, procedures, clinical outcomes, emergency care, or 24/7 response. Nonurgent messaging and priority scheduling depend on staffing and availability. Additional fees, exclusions, visit definitions, and no-show rules are specified in the signed plan schedule.
Before enrollment, patients must receive the complete patient membership agreement, approved plan-specific benefits and exclusions schedule, privacy and communication notices, and recurring payment authorization. In case of differences, the executed agreement and applicable law control.
