Membership Terms
Essential, Plus, Premier and Total Vitality require a 3-month initial commitment, then continue month-to-month. Prime Pellet Complete requires an initial 6-month commitment because treatment is longitudinal and procedure-based. Auto-pay is required for all memberships. Membership is not health insurance and does not guarantee a prescription, procedure, medication, diagnosis, or clinical outcome.
What Isn’t Included Unless Specifically Stated
Laboratory draw/testing charges; imaging or pathology; outside specialists; urgent/emergency or hospital care; prescription or compounded medication costs; supplements; IV therapy; aesthetic services; and services outside the selected membership. Clinical care unrelated to the membership may be billed separately.

Cancellation & Payment Policy
- Essential, Plus, Premier and Total Vitality require an initial 3-month commitment. After the initial term, membership continues month-to-month until canceled.
- Prime Pellet Complete requires an initial 6-month commitment because treatment is longitudinal and procedure-based.
- Cancellation must be submitted in writing at least 30 days before the next renewal/payment date. Cancellation becomes effective at the end of the applicable paid membership period.
- Membership fees already paid are non-refundable and are not prorated for unused benefits, except when otherwise required by law or approved by Prime Vitality.
- During an initial commitment period, the remaining committed payments remain due unless Prime Vitality approves an exception or applicable law requires otherwise.
- Membership benefits may be suspended for failed or past-due payments. Repeated failed payments may result in termination.
- Unused monthly benefits do not roll over and are non-transferable.
Clinical & Safety Policies
- All prescriptions, hormone therapy, pellet procedures and weight-management treatment require provider evaluation and must remain medically appropriate.
- Treatment may be delayed, adjusted or declined because of abnormal laboratory results, pregnancy, contraindications, medication interactions, uncontrolled medical conditions or other safety concerns.
- HRT and pellet patients receive individualized dosing and timing. No exact reinsertion date, symptom response or outcome is guaranteed.
- Prime Vitality will obtain treatment-specific informed consent when appropriate. Pellet insertion has procedure-related risks that are reviewed before treatment.
- Non-urgent messages are addressed during normal business hours. Urgent symptoms require urgent care or emergency services; call 911 for an emergency.
- Membership does not replace a primary-care relationship, specialist care, health insurance or emergency care when those services are needed.

What is the Prime Vitality Membership Plan?
It's a 2026 membership program with four general wellness tiers — Essential, Plus, Premier and Total Vitality — plus three focused treatment memberships (Prime Hormone Optimization, Prime Pellet Complete, and Prime Medical Weight Management) for patients whose primary goal is hormone or weight-management treatment.
What's the difference between a wellness tier and a focused treatment membership?
The four wellness tiers (Essential through Total Vitality) bundle provider visits, preventive care, and body-composition tracking, with higher tiers adding weight- and hormone-management oversight. A focused treatment membership — hormone optimization, pellet therapy, or medical weight management — is built specifically around that single treatment goal instead.
Does membership replace insurance?
No. Membership is not health insurance and does not guarantee a prescription, procedure, medication, diagnosis, or clinical outcome. It covers the specific visits and services included in your selected plan — it does not cover emergency care, hospitalization, or services outside your plan.
Is lab testing included?
Laboratory draw/testing charges are separate unless a written package specifically includes them, as are imaging, outside specialists, medication and pharmacy costs, supplements, and services outside the selected membership.
Can I cancel my membership?
Yes. Essential, Plus, Premier and Total Vitality require an initial 3-month commitment (6 months for Prime Pellet Complete), then continue month-to-month. Cancellation must be submitted in writing at least 30 days before your next renewal date and takes effect at the end of the paid membership period; fees already paid are non-refundable.
Is this available anywhere in Nevada?
Yes. As a Nevada telehealth practice, membership is available statewide, with pellet-therapy procedures and any other in-person components scheduled accordingly.
